A mechanic was cleaning an electric fork lift with a cleaning solvent that exploded. The mechanic fell backwards, and injured his back. This resulted in a long-term disability.

The Chemaxx role was to assess the flammability of the solvents in question. Through experimentation, it was demonstrated that relatively non-flammable solvents (such as mineral oil) become highly flammable when sprayed as a fine mist. Other issues included the Material Safety Data Sheet and labeling.
